SCHOOL STARTS SOON GET YOUR KIDS IMMUNIZED

Florida Department of Health in Palm Beach County is sending out a reminder to all parents and care givers that Palm Beach County Schools begin August 15, 2016 and new incoming students will need their immunizations up to date.

Appointments and the required DH 680 form are available at most medical provider offices, walk-in clinics and Department of Health in Palm Beach County. The required immunizations include, mumps, measles, rubella, (MMR), chickenpox, Hepatitis B, Diphtheria, Tetanus and polio.

In addition to the regularly scheduled school immunizations at the health department clinics in Lantana, Belle Glade, Jupiter, Lantana, Delray Beach and West Palm Beach, the immunization van will visit Lake Worth High School on August 4, 9:30 a.m.-1:30 p.m. The last outreach before school will take place Saturday, August 13, at the West Palm Beach Health Center, 1150 45th Street, from 9 a.m. to 2 p.m. while supplies last.

Florida Certificate of Immunization, Form DH 680 is the only document that the schools are permitted to accept as proof of immunization.

Kindergarten to Grade 12

Diphtheria/Tetanus/Pertussis (DTaP) – 5 doses

Polio (OPV or IPV) – 4 doses (the last one after age 4)

Measles, Mumps, Rubella – 2 doses (usually given as 2 doses MMR)

Hepatitis B (Hep B) – 3 doses

Kindergarten to Grade 8

Varicella – 2 doses or provider verification of Chickenpox disease history on form DH 680

Grades 9 to 12

Varicella – 1 dose or provider verification of Chickenpox disease history on form DH 680

Grades 7 to 12 (entry or repeating, if not given previously)

Tetanus/Diphtheria & Pertussis booster (Tdap) – 1 dose
