Palm Beach, Fla. — The Florida Department of Health in Palm Beach County (DOH-Palm Beach) is expanding its services to better serve the community, with the Delray Beach Health Center now offering notary services.
Residents can visit DOH-Palm Beach Vital Statistics offices at the following locations to obtain notary services along with birth and death certificates:
- Main Office: 800 Clematis St., West Plam Beach, FL 33401
- Delray Beach Health Clinic: 225 South Congress Ave., Delray Beach, FL 33445
DOH-Palm Beach is the only entity in Palm Beach County providing this combined service. Customers can complete both tasks in one visit, saving time and effort. The notary fee is $10.
About Notary Services
The main office has offered notary services since 2017, meeting high daily demand. In response to community requests, the Delray Beach Health Center began offering notary services in October 2025.

- Titles
- School letters
To have a document notarized, bring the complete, unsigned original document and a valid government-issued photo ID.
