- Make sure to bring your child’s immunization records with you.
- Walk-in appointments are on a first come, first served basis.
- To schedule an appointment at any of the Florida Department of Health Palm Beach County Health Centers, call 561-625-5180
- To request immunization forms or transfer immunization records, visit the Florida Department of Health Immunization Portal
