Surveillance

Reports must be submitted to the county health department and must contain the following:

1. Patient name, and aliases, if any
2. Patient address-including city, state, and zip code
3. Patient contact number (e.g., telephone, cell phone, etc.)
4. Date of birth
5. Sex
6. Race and ethnicity
7. Social Security Number
8. Risk information-mode of transmission
9. HIV test date (specimen collection date)
10. CD4 count
11. Viral load
12. Country of birth
13. Patient notification status (comment section)
14. Treatment/services referrals
15. Provider name
16. Provider name and address-including city and state
17. Name and phone number of person completing form
18. Pregnancy status (if applicable)
19. Incidence/resistance information
20. HIV-exposed newborns (0-18 months) must be reported on the Pediatric HIV/AIDS Confidential Case Report (call the office if you have an exposed newborn)

Section 384.25, F.S. – Sexually transmissible diseases, reporting required

Public Health Reporting is Exempt from HIPAA – According to the U.S. Health Information Portability and Accountability Act of 1996 (HIPAA) regulations, 45 CFR 160.203, disease reporting, public health disease surveillance, and disease intervention activities are among those that are EXEMPT from federal preemption of state laws. Other examples include child abuse, and birth or death information.
